Scheduling Surgery Print E-mail
Prior to scheduling your surgery, you will need to complete a medical history form and sign a surgery consent form.  Although VOA will assist you to determine insurance coverage for your surgery, you are encouraged to contact your insurance provider about your specific benefits and coverage including those for anesthesia.

If your surgery is NON-ELECTIVE:

  • You will meet with a surgery scheduler to choose a date for your procedure.
  • You will be advised if you need to see your primary care physician prior to surgery and given a brochure about VOA Ambulatory Surgery Center.
  • The VOA authorization department will contact your insurance provider to obtain authorization for your procedure.
  • If medical or cardiac clearance is required before your surgery, a surgery date will NOT be scheduled until clearance is received.
  • A surgery scheduler will contact you one working day prior to your surgery to inform you of your check-in time.  If you do not receive a call, please contact surgery scheduling at 425-656-5060, ext. 3062.

If your surgery is ELECTIVE:

  • The VOA referral and authorization department will contact your insurance provider to determine coverage and benefits for your procedure.
  • The VOA financial (bookkeeping) department will determine any financial responsibility you may owe for your procedure after your insurance provider is billed.  If it is determined you will have out-of-pocket expenses for your procedure, you will be contacted to make a pre-payment based on your expected financial responsibility.  Your surgery date will NOT be scheduled until pre-payment is received.  If no out-of-pocket expenses are expected for your procedure, you will NOT be contacted by bookkeeping.
  • After payment verification is determined, you will be contacted by a surgery scheduler to select a date for your procedure.  The scheduler will advise if you need to see your primary care physician prior to surgery and provide information about pre- and post-operative procedures.
  • If medical or cardiac clearance is required before your surgery, a surgery date will NOT be scheduled until clearance is received.
  • Please Note: This process may take five to seven business days.
  • A surgery scheduler will contact you one working day prior to your surgery to inform you of your check-in time.  If you do not receive a call, please contact surgery scheduling at 425-656-5060, ext. 3062.

If your surgery is covered under WORKERS' COMPENSATION:

  • The VOA referral and authorization department will contact DLI or your SI Company to obtain authorization - this may take two to three weeks.
  • A surgery scheduler will contact you to select a date for your procedure and advise if you need to see your primary care physician prior to surgery, as well as provide information about pre- and post-operative procedures.
  • A surgery scheduler will contact you one working day prior to your surgery to inform you of your check-in time.  If you do not receive a call, please contact surgery scheduling at 425-656-5060, ext. 3062.
